Published in the January 2011 Texas Board of Nursing Bulletin, Volume 42, No. 1On or about May 27, 2010, Candace Ruth Cobb submitted a Texas Online Examination Application for vocational nurse to the office of the Board and was issued a temporary permit to practice. Also received was a copy of a Diploma from a college in Corsicana, Texas, which was not the school Ms. Cobb listed for her education. This document was later reported to be fraudulent. On July 20, 2010, the Board office received an Affidavit of Graduation from a school in Hanford, California. This document was also fraudulent. The document contained a forged signature, and the Board received verification that Ms. Cobb did not attend or complete a nursing program from the school.
In addition, Ms. Cobb submitted an application for employment as a GVN to a long term care facility in Corsicana, Texas, and indicated that she graduated from a college in Dallas, Texas. On August 17, 2010, Ms. Cobb secured employment as a GVN and worked with the facility until September 15, 2010, when she was terminated. The temporary permit issued to Candace Ruth Cobb was rescinded and she was denied licensure by examination. The case was referred to the Travis County District Attorneys Office, Austin, Texas. |
