Jamie Marie Ontiveros

On August 15, 2011, Jamie Marie Ontiveros voluntarily surrendered her license to practice vocational nursing in the State of Texas. Ms. Ontiveros secured employment as an LVN with a staffing agency in Odessa, Texas. She worked as an LVN from August 31, 2011 through April 20, 2012, at which time the agency discovered that her license was not valid. Ms. Ontiveros then secured employment as an LVN with a long term care facility in Crane, Texas. She worked for the facility without a valid license from April 24, 2012 through June 22, 2012. The case was referred to the Ector County Attorneys Office, Odessa, Texas, and to the Crane County District Attorneys Office, Odessa, Texas.

Gloria Ann Mederos

On February 13, 2013, Gloria Ann Mederos submitted an Application for Employment as a LVN to a long term care facility in Jasper, Texas. With the application she also submitted a copy of her social security card, drivers license and a copy of a Board of Nursing certificate with her name on it. During the verification process the facility found that the LVN license number that Ms. Mederos provided belonged to another individual who was a former employee of the facility. Facility staff confronted Ms. Mederos and she said it must be a mistake and that she would get it straightened out. No offer of employment was extended.
