Leticia Gallarzo secured employment as a Registered Nurse (RN) in Victoria County, Texas, as well as with a long term care facility in Goliad, Texas. Ms. Gallarzo is not licensed to practice professional nursing in the State of Texas; however, she used the name and RN license number belonging to a licensee with a similar name, but a different social security number who resides in the Hidalgo County area. In addition, the Board has received information that this individual has worked as a RN in DeWitt County, Victoria County, Goliad County, and Matagorda County. This case has been referred to the Victoria County District Attorney’s Office and the Goliad County District Attorney’s Office

 

Nursing Imposter: SHEILA MARIE VINCENT

Sheila Marie Eyre Vincent had been employed as a Registered Nurse with a clinic in College Station Texas, since 2006. On January 16, 2015, the clinic staff found that the license number Ms. Vincent was using belonged to another licensee. Ms. Vincent was placed on administrative leave on January 19, 2015, pending investigation. On January 21, 2015, Ms. Vincent emailed the clinic a resignation. Sheila Marie Eyre Vincent is not licensed to practice professional nursing in the State of Texas and she had presented forged documents to secure the position of RN. She was using a license number belonging to a licensee who resides in the Dallas area. Sheila Marie Eyre Vincent was arrested January 26, 2015 and then later turned herself in to the Brazos County Sheriff’s Department. She was charged with a third-degree felony forgery of a government instrument and fraudulent use of identifying information, a state jail felony. The case is pending in Brazos County.

 

Nursing Imposter::OKIMBERLY ANN JOHNSON

KIMBERLY ANN JOHNSON (SFAKIANOS)

On May 13, 2014, the Board revoked the Privilege to Practice (PTP) from Mississippi belonging to Kimberly Ann Johnson (Sfakianos). On August 8, 2014, Kimberly Ann Johnson (Sfakianos) applied for employment with a home health agency in McKinney, Texas. Respondent provided a license number belonging to another licensee with a similar name, but different social security number who resides in Houston, Texas. Kimberly Ann Johnson (Sfakianos) was hired and she began working for the agency on August 11, 2014, conducting home health visits for patients. On September 2, 2014, the agency received a phone call reporting that Respondent had her license (PTP) revoked. The agency contacted the Board and verified that the license number provided by Respondent belonged to a different licensee. Respondent was terminated from employment. The case was referred to the McKinney Police Department.

The April 2015 issue of the Board of Nursing Bulletin reported on the illegal practice of nursing by Kimberly Ann Johnson (Sfakianos).  Ms. Johnson (Sfakianos) had her Privilege to Practice from Mississippi revoked by the Texas Board of Nursing (BON) on May 13, 2014.   She continued to practice in Texas utilizing the identity of other licensees with similar names.  Ms. Johnson (Sfakianos) was arrested by the McKinney Police Department on April 28, 2015 for the offenses of Fraudulent Use or Possession of Identifying Information, a State Jail Felony, and Prohibited Practice, Nurse License Required, a Class A Misdemeanor. The case is pending in Collin County.  The BON has also received information from several agencies that Ms. Johnson (Sfakianos) has continued to seek employment and has practiced as a registered nurse in Dallas and Carrollton using information belonging to other licensees.
