Margaret Juliet Musa secured employed as a licensed vocational nurse (LVN) with a home health agency in Temple, Texas, on August 11, 2011. Ms. Musa is not licensed to practice nursing in any state. Ms. Musa used the name of an LVN from Virginia and also used the LVN’s license with Privilege to Practice from the State of Virginia. Ms. Musa was employed as a vocational nurse with the agency performing skilled nursing care for pediatric patients, from August 11, 2011 through December 20, 2013. The case involving Ms. Musa is under investigation by the Temple Police Department for the offense of Fraudulently Practicing Nursing.

Leticia Gallarzo secured employment as a registered nurse (RN) with a staffing agency in Victoria County, as well as with long term care facilities in Goliad and Yorktown. Ms. Gallarzo is not licensed to practice professional nursing in the State of Texas, however she used the name and RN license number belonging to a licensee with a similar name, but different social security number who resides in the Hidalgo County area. In addition, the Board of Nursing has received information that this individual has worked as a RN in DeWitt, Victoria, Goliad, and Matagorda Counties. This case has been referred to the Victoria County District Attorney’s Office, Goliad County District Attorney’s Office, and DeWitt County District Attorneys' Office.

Ms. Esquivel’s employment was terminated after her employer verified her license using the Board’s website and found that her license had been in surrendered status since August 28, 2015. The case has been referred to the Midland County District Attorney’s Office. |

Tammie D. Hailey secured employment and practiced as a Licensed Vocational Nurse with a home health agency in Richardson, Texas, between December 7, 2015 and December 10, 2015. When a patient decided to transfer care to a different home health agency, Ms. Hailey submitted an application for employment with the new home health agency, who then discovered Ms. Hailey was not licensed to practice vocational nursing in the State of Texas. This case has been referred to the Cass County District Attorney’s Office. |

Juan Manuel Perez secured employment and practiced as a Licensed Vocational Nurse at a local hospital in Harlingen, Texas, between June 2014 and November 2015 by using the license number belonging to another nurse with the same name. In addition, Mr. Perez presented the facility with a copy of a certificate showing that he had completed an educational program in vocational nursing from an accredited university; however, when the facility tried to verify the certificate with the university’s registrar, the registrar indicated there was no record of Juan Manual Perez completing a vocational nursing program. The facility terminated the employment of Juan Manuel Perez. The case has been referred to the Cameron County District Attorney's Office. |

Topangelia Shaw gained employment and practiced as a Registered Nurse with a home health agency in Dallas, Texas, between March 2, 2015 through July 16, 2015, and with a home health agency in Fort Worth, Texas, between April 14, 2015 and June 26, 2015, using a registered nurse license number belonging to a nurse with a completely different name. When asked for additional information in order to verify her license, Ms. Shaw provided the name of a different nurse whose name was similar to Topangelia Shaw. Ms. Shaw does not hold a license to practice professional nursing in the State of Texas. The cases have been referred to the Dallas District Attorney’s Office and to the Tarrant County District Attorney’s Office. |

Mr. Gallagher secured employment at a skilled nursing facility, although not in a position requiring a license to practice nursing, and used the credential on his business cards, via email correspondence, and on a LinkedIn account. Mr. Gallagher is no longer employed with the facility. The case was referred to the Bexar County District Attorney’s Office. |
