On or about August 19, 2014 through February 2018, Antoinette Sue Baudry obtained employment with an orthopedic clinic in Houston, Texas. Ms. Baudry represented herself as a licensed vocational nurse (LVN), and used “LVN” credentials when she does not hold a license to practice nursing in the State of Texas. The case was referred to the Harris County District Attorney’s Office.

Kristi Jones-Granito

a.k.a. Kristi Lynn-Ann Granito; a.k.a. Kristi Jones; a.k.a. Kristi Granito

On or about April 5, through May 16, 2018, Kristi Jones-Granito secured employment and fraudulently worked as a registered nurse (RN) at a correctional facility in Wichita Falls, Texas, by using the RN licensure information of a nurse having the same name and submitting a resume which falsely indicated she had a license to practice professional registered nursing in the State of Texas. The Board’s investigation revealed that the date of birth, social security number, and address provided by Kristi Jones-Granito to the facility did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Office of the Wichita County District Attorney for prosecution.

Kimyaida Hamilton-Rollins

Kimyaida Hamilton-Rollins

a.k.a. Kimyaida Hamilton; a.k.a. Kimyaida Rollins; a.k.a. Kimyaida Evon Rollins; a.k.a. Kimyaida Evon Hamilton

On or about August 30, 2017 through January 5, 2018, Kimyaida Hamilton-Rollins secured employment and worked as a Graduate Vocational Nurse (GVN) at a nursing home in Benbrook, Texas. Kimyaida Hamilton-Rollins was also the staffing coordinator during the same time period. The Board’s investigation revealed that Kimyaida Hamilton-Rollins received a certificate in nursing from Fortis College, Grand Prairie, Texas, but was never issued a temporary permit to work as a GVN by the Texas Board of Nursing. The case has been referred to the Office of the Tarrant County District Attorney for prosecution.

Crystal Moya

On or about September 16, 2016, through July 21, 2017, Crystal Mascorro Moya secured employment and fraudulently worked as a licensed vocational nurse at a women’s clinic in the Corpus Christi, Texas, area by falsely representing to the employer that she had recently obtained a license to practice vocational nursing in the State of Texas and that it would take up to 10 business days to update the Board’s licensure information. When Crystal Mascorro Moya did not respond to the employer’s repeated requests for licensure information, the employer accessed the Board’s online licensure verification system and discovered that Crystal Mascorro Moya did not have a license to practice vocational nursing in the State of Texas.  According to the Board of Nursing records, Crystal Mascorro Moya has never been issued a license and does not possess a privilege to practice vocational nursing in the State of Texas. The case was referred to the Nueces County District Attorney for prosecution.

a.k.a. Tokunbo Mary Are; a.k.a. Tokunbo Are; a.k.a. Tokunboh Are; a.k.a. Mary Are; a.k.a. Jamail R. Are; a.k.a. Jamail Rashoun Are; a.k.a. Jamail Are; a.k.a. Mary Are    

In August 2017, Tokunboh Mary Are secured employment as a licensed vocational nurse with a home health agency in Kaufman County, Texas, using licensure information which belonged to a currently licensed vocational nurse with a similar name. Tokunboh Mary Are is not licensed to practice vocational nursing in the State of Texas, and the home health agency became suspicious because of  discrepancies in the various documents provided by Tokunboh Mary Are for identification.

Ashley Nicole Pettis
a.k.a. Ashley Nicole Jones

Between May 10, 2017, and October 4, 2017, Ashley Nicole Pettis, a.k.a. Ashley Nicole Jones, remained employed with a staffing agency in the Fort Worth, Texas area after her license had been revoked by the Board.  Ashley Nicole Pettis continued to practice as a licensed vocational nurse providing hospice care to patients at facilities and in their homes.  According to the Board’s licensure records, the license of Ashley Nicole Pettis to practice nursing in the State of Texas was revoked on May 9, 2017. The case has been referred to the Tarrant County District Attorney for prosecution.

Leticia Gallarzo
a.k.a. n/a

Imposter alerts regarding the unauthorized practice of registered nursing by Leticia Gallarzo were originally published in the October 2015 and April 2016 editions of the Texas Board of Nursing Bulletin.  On July 6, 2017, Leticia Gallarzo plead guilty to five counts of knowingly making false statements related to health care for falsely representing herself as a registered nurse at two hospitals and three skilled nursing facilities in five different cities in southern Texas between March 2015 and October 2015. On October 3, 2017, Leticia Gallarzo was sentenced to serve 14 months in the custody of the United States Bureau of Prisons, to be followed by 3 years of supervised release, and was ordered to pay more than $34,000 in restitution.

Stephanie Garcia
a.k.a. Stephanie Halcomb

In May 2016, Stephanie Garcia, also known as Stephanie Halcomb, obtained employment as a registered nurse  (RN) at a dialysis facility in the Marshall, Texas, area using the RN licensure information of a nurse having the same name and by submitting an application which contained falsified employment history as a RN dating back to August 2011.  In May 2017 through October 2017, Stephanie Garcia was promoted as a registered nurse and practiced as the facility administrator.  The Board’s investigation revealed that the date of birth, social security number, and address information provided by Stephanie Garcia to the facility did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Office of the Harrison County District Attorney for prosecution.

Lydia Gonzales

On August 4, 2017, Lydia Gonzales attempted to gain employment as a RN with a home health agency in the Houston, Texas, area, by submitting an application which falsely indicated she had a license to practice professional registered nursing in the State of Texas. The application included the license number issued to a registered nurse of the same name. The home health agency staff were alerted to the attempt when they tried to verify licensure using the last four digits of the social security number and the date of birth provided by Lydia Gonzales. The investigation conducted by the Board revealed that Lydia Gonzales had previously been a licensed vocational nurse whose license to practice nursing has been in revoked status since September 13, 2016. The case has been referred to the Office of the Harris County District Attorney for prosecution.

Christopher Ross Thompson

From October 2013 through September 2017, Christopher Ross Thompson continued to practice as a registered nurse at a behavioral health transport company in the Dallas, Texas, area after his license had been revoked by submitting to the company altered and forged RN licensure verifications which appeared to be from the Board.  According to the Board’s licensure records, the license of Christopher Ross Thompson to practice nursing in the State of Texas has been revoked since October 17, 2013.  The case has been referred to the Dallas County District Attorney for prosecution.
