On or about September 9, 2019, through October 16, 2019, Nora Nely Avila (Ibarra) secured employment and practiced as a registered nurse (RN) with a home health agency in Laredo, Texas, by using the license number belonging to another nurse with the same first and last name, but with a different middle name. During this time, Nora Nely Avila represented herself as a registered nurse to staff, patients, and the public while using the credentials of “RN” on the facility’s business and medical records. The Board’s investigation revealed that the date of birth, social security number, and address information provided by Nora Nely Avila to the facility did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Office of the Webb County District Attorney for prosecution.

Jessica Ayala Lopez:
a.k.a Jessica Lopez; Jessica A Lopez

Between January 9, 2017, and February 12, 2020, Jessica Ayala Lopez secured employment and practiced as a registered nurse (RN) with two (2) staffing agencies, three (3) surgical centers, and one (1) hospital in Harris County, Texas, by using a license number belonging to another nurse with a different first and last name. Additionally, Jessica Ayala Lopez provided a fraudulent paper license to employers with a fake license number indicating she was a registered nurse. During this time, Jessica Ayala Lopez represented herself as an RN to staff, patients, physicians, and the general public while using the credentials of “RN” on facility’s business and medical records. The Board’s investigation revealed that the date of birth, social security number, and address information provided by Jessica Ayala Lopez to the facilities did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Office of the Harris County District Attorney for prosecution.

NORMA ALICIA SOLIS:
a.k.a Norma A. Solis; a.k.a. Norma Solis; a.k.a. Norma Solis Solomon; a.k.a. Norma Solomon

On or about May 6, 2018, through April 28, 2019, Norma Alicia Solis secured employment as a medical assistant (MA) with an urgent care pediatric facility in Dallas, Texas. During her employment, she misrepresented herself as a nurse to staff and patients as well as practiced beyond the scope of nursing by diagnosing, writing prognoses, and advising patients on plans of care. Further, on or about March 11, 2019, through April 10, 2019, Norma Alicia Solis was promoted to Practice Director of a wound care clinic in Dallas, Texas, a position which required licensure as a registered nurse. Norma Alicia Solis represented herself as an actual licensee with the same first and last name, but with a middle name spelled differently. The Board’s investigation revealed that the date of birth and social security number provided by Norma Alicia Solis to employers did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Office of the Dallas County District Attorney for prosecution.

Between March 30, 2016, and December 16, 2019, Liliana Barrera Amador secured employment and practiced as both a licensed vocational nurse (LVN) and registered nurse (RN) with multiple hospice and staffing agencies in Houston, Texas, by using license information belonging to a licensee with the same first and last name. During this time, Liliana Barrera Amador represented herself as a “LVN" and “RN” to staff, patients, physicians, and the general public while using the credentials of “LVN” and “RN” on medical and business records. The Board’s investigation revealed that date of birth, social security number, state issued driver's license number, and address information provided by Liliana Barrera Amador to her multiple employers did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. This case has been referred to the Office of the Harris County District Attorney for prosecution.

On or about March 7, 2019, through May 15, 2019, Marilyn Nunn Juarez failed to inform her employer that her license had been revoked by the Board on March 5, 2019, and continued to practice as a licensed vocational nurse (LVN) with a home health agency in Austin, Texas. During this time, Marilyn Nunn Juarez continued to represent herself as a LVN to staff, patients, physicians, and the public while using the credentials of “LVN” on medical records and various other forms. The case has been referred to the Office of the Travis County District Attorney for prosecution.

Julia Silva Perez secured employment and practiced as a licensed vocational nurse (LVN) with a gastroenterology practice in San Antonio, Texas, from November 20, 2017, through May 7, 2018, and with a rehabilitation center in San Antonio, Texas, from August 1, 2018, through November 22, 2018, by using the license number belonging another nurse with the same first and last name. During this time, Julia Silva Perez represented herself as a LVN to staff, patients, physicians, and the public while using the credentials of “LVN” on medical and various other records. Further, on October 23, 2018, Julia Silva Perez attempted to secure employment as a LVN with a healthcare staffing agency in San Antonio, Texas, by using the license number belonging to another nurse with the same first and last name. Management at the staffing agency were alerted to the attempt when they tried to verify the license using the information provided by Julia Silva Perez. The Board’s investigation revealed that the date of birth, social security number, and address information provided by Julia Silva Perez to employers did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Bexar County District Attorney for prosecution.

Pamela Jo Trevino secured employment and practiced as a licensed vocational nurse (LVN) with a home health agency in Boerne, Texas, from September 24, 2019, through October 2, 2019, by using the license number belonging to another nurse with the same first and last name. During this time, Pamela Jo Trevino represented herself as a LVN to staff, patients, and the public while using the credentials of “LVN” on medical and business records. The Board’s investigation revealed that the date of birth, social security number, and address information provided by Pamela Jo Trevino to her employer did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. The case has been referred to the Office of the Kendall County District Attorney for prosecution.

On or about March 19, 2019, through June 21, 2019, Glenda Tate Basanes obtained employment and practiced as a registered nurse with a nursing home in Collin County, Texas. The Board’s investigation revealed that the date of birth, social security number, and address provided by Glenda Tate Basanes to the employer did not belong to any nurse having a license or privilege to practice nursing in the State of Texas. Glenda Tate Basanes is currently facing criminal charges in Collin County, Texas.

Stephanie Garcia

An imposter alert regarding the unauthorized practice of registered nursing by Stephanie Garcia was originally published in the January 2018 edition of the Texas Board of Nursing Bulletin. On November 12, 2019, Ms. Garcia plead guilty to one felony charge and two misdemeanor charges related to obtaining employment as a registered nurse at a dialysis facility in the Marshall, Texas, area. On December 11, 2019, Judge Brad Morin sentenced Ms. Garcia to 20 months state-jail custody.
